LMGVB REGISTRATION & PHYSICAL EXAM INSTRUCTIONS
Before student-athletes can participate in volleyball practices, scrimmages, or games at Lower Merion High School—or any Pennsylvania Interscholastic Athletic Association (PIAA) member school—they must complete registration through the ArbiterSports FamilyID online platform. This platform collects medical history, emergency contacts, and the Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) form.
Important: Students will not be permitted to try out, practice, or compete without completing the CIPPE form and online registration. The athletic department does not accept paper physicals—all completed and approved physicals must be submitted online each school year.
Health History – Records past injuries, medical conditions, and family medical history.
Physical Examination – A medical professional evaluates height, weight, blood pressure, vision, heart, lungs, and musculoskeletal health.
Clearance Section – Determines if the athlete is medically cleared to participate.
Emergency Information – Provides essential contact and medical details in case of emergencies.
Consent & Acknowledgments – Requires parent/guardian and athlete signatures to confirm understanding of risks and responsibilities.
Visit the Family ID website and log in or create an account.
Select Lower Merion High School and find the volleyball registration for the upcoming season.
Fill out all required information, including medical history and emergency contacts.
Download and print Section 6 of the CIPPE form. Two ways to access the CIPPE Forms and Section 6:
Schedule a sports physical with an Authorized Medical Examiner (doctor, nurse practitioner, or physician assistant).
Have the AME complete, sign, date, and stamp Section 6. The stamp verifies the physical is valid.
Important: Only Section 6 must be printed and completed by an AME. No other forms from the doctor will be accepted.
Log in to ArbiterSports and go to the upload section (eFiles, Documents, or Forms).
Upload the scanned Section 6 form:
Select Document Type – Choose "PIAA Section 6" or similar.
Attach the File – Ensure it is clear and legible.
Check File Size – Some platforms may have file size limits (e.g., under 1MB).
Save and confirm the upload. Check for confirmation messages or emails.
Complete all remaining online registration steps, including required parent/guardian and student signatures.
✔ Log in to ArbiterSports to confirm that all documents are uploaded and approved.
✔ If there are any issues, you will receive a notification to correct them before the deadline.
